The following forms are required to be on file for students wishing to apply for the initial use of their VA Educational Benefits at MCC. Please print and complete each form below.

Veterans and Dependents (Chapter 30, 35, 1606, & 1607)

  1. OVA-1
  2. OVA-2A
  3. OVA-3
  4. OVA-4 (Form is pending 1/11/07 - Please disregard for now.)
  5. OVA-5
  6. OVA-6 (This form is to defer your tuition.  It is not required, it is a benefit for those wishing to use it. Please see the VA Certifying Official or Financial Aid Advisor for assistance)
  7. OVA-7 (Your Academic Counselor must sign this form before submitting it to MCC)
  8. OVA-8 (You must complete this form each semester that you are enrolled in a zero-level course)

VA Vocational Rehabilitation Students (Chapter 31)

  1. OVA-1
  4. OVA-4 (Form is pending 1/11/07 - Please disregard for now.)
  6. OVA-7 (Your Academic Counselor must sign this form before submitting it to MCC)

  7. OVA-8 (You must complete this form each semester that you are enrolled in a zero-level course) 

VA Vocational Rehabilitation students must have their State VA Vocational Rehabilitation Counselor submit a VA Form 28-1905 directly to the MCC VA Certifying Official.

Note: All continuing MCC students are required to submit an OVA-1 and an OVA-7 each semester they wish to apply for benefits. Students must also submit an OVA-8 if applicable.
