Attention Deficit/Hyperactivity Disorder - A formal psychological or neuropsychological assessment which demonstrates impaired attention and/or hyperactivity must be provided.

A behavioral history consistent with a diagnosis of AD/HD requires an emergence of symptoms in childhood and must be differentiated from other causes of attention and/or hyperactive difficulties and must be shown to have caused significant impairment in social, academic, or occupational functioning. Prior assessment must be included if it established the diagnosis. Psychometric measures of aptitude and academic achievement are required.

Assessment of the following areas by a certified or licensed professional, e.g.; psychologist, neuropsychologist, school psychologist, or state certified educational diagnostician:

  • History - Clinical interview to determine history of attention problems from childhood with history of educational problems related to attention deficits.
  • Assessment of Intellectual Function - Intellectual functioning which demonstrates average or above average intelligence.
    • A screen of intellectual functioning is not acceptable WAIS III or Stanford - Binet 4th edition are recommended.
    • Raven is accepted it if can be demonstrated that use of the above tests are contraindicated due to severe language deficits
    • Include scaled scores of all sub-tests
  • Achievement Assessment - Individually administered measure of level of achievement
    • A screen of level of achievement is not acceptable.
    • Woodcock-Johnson Psychoeducational Battery – revised, or the WIAT are recommended: or any combination of instruments determining current achievement in reading, written language and mathematics.
  • Attention - Assessment of attention factors is strongly recommended (a continuous performance test such as the TOVA or Connors)
  • Personality - Personality inventory such as the MMPI-2 to rule out other possible causes of attention issues. A detailed report from clinical interview observations will be accepted in lieu of MMPI-2.
  • Report - A full signed, written report is required and must include the following:
    • Dates of assessment
    • Names, titles, certification and/or license numbers
    • Summary of results
    • DSM-IV diagnosis
    • Statement of functional limitations with related recommendations
    • Current treatment
    • Psychometric summary of standard scores
