Suggested Documentation*

Traumatic Brain Injury (TBI) - Current neuropsychological assessment by qualified neuropyschologist at least one year post injury with a DSM-IV or ICD-10 diagnostic statement and functional limitations with related recommendations as well as current treatment.

Blind/Visual Impairment - Medically certified loss by an ophthalmologist or MD listing functional limitations.

  1. The visual acuity of 20/70 or less in the better eye after correction
  2. A peripheral field so constricted that it affects one's ability to function in an educational setting.
  3. A progressive loss of vision that may affect one's ability to function in an educational setting.

Deaf/Hard of Hearing - Audiogram by certified audiologist with listing functional limitations. A hearing loss of 30 decibels or greater, pure tone average of 500, 1000, 2000 HZ unaided in the better ear.

Physical Disability - Medical documentation from a qualified professional with a diagnosis and functional limitations. Physical conditions that may require an adaption to one's school environment or curriculum.

Psychological Disability - Psychological evaluation or treatment summary by licensed psychologist or psychiatrist within the last 12 months with DSM diagnosis including a global assessment of functioning, list of functional limitations and current treatment, or completed Mohave Community College Psychiatric or ADD verification form.

Temporary Impairments - Some students have temporary impairments and may require accommodations for a limited time. Each case is considered individually. The following documentation is required. Letter on letterhead from a qualified professional stating diagnosis, functional limitations necessitating the accommodation and estimated period of time services will be needed.

Chronic Impairments - Assessment by qualified professional with a written report that includes an ICD-10 diagnosis, functional limitations that affect a major life activity, functional limitations related to school and recommendations.*

*If your documentation does not meet the guidelines above, Mohave Community College's Director of Disability Services may review the documentation to determine if it is sufficient to receive services.
